Sample Document Checklist for Naturalization Application (N-400)

1. Copy (front and back) of Permanent Resident Card (Green Card);

2. If you have changed your name since becoming a U.S. Permanent Resident, provide a copy of your Marriage Certificate, Court Order for Name Change, Adoption Decree, or other name-change document;

3. If you have taken any trips outside the United States since becoming a U.S. Permanent Resident, provide:

a. Copy of entire passport for current passport(s);

b. Copy of entire passport for other passports held since becoming a U.S. Permanent Resident;

c. Flight itinerary emails (if available);

4. Copy of Drivers License or State ID;

5. Copy of Marriage Certificate;

6. If you were previously married, copies of documentation of the termination of previous marriage(s), such as Divorce Decree(s), Annulment Decree(s), or Death Certificate of prior spouse;

7. If you have had dependent children that did not live with you, evidence that you provided financial support to the dependents.

8. If you are applying as the spouse of a U.S. citizen, please also provide:

a. Proof that you have been living with your U.S. citizen spouse for the past three years (i.e., joint lease, joint bank accounts, joint utility bills, joint tax returns, driver’s licenses showing same address, etc.);

9. IRS Tax Return Transcripts for the past three years. Please download them directly from the IRS website here: https://www.irs.gov/individuals/get-transcript

10. If you have EVER been arrested or charged with any crime or violating any law, please provide:

a. Certified police records for ANY arrests or citations; AND

b. Certified Court Disposition for ANY arrests;

c. If the police and/or court have no record of the charge or citation, have them provide you with a certified letter or notice stating that no record exists.
